Synopsis

Severe treatment refractory OCD implies an individual has failed one or more trials of CBT. This means judging the quality of any previous trials of CBT, or previous interfering factors or not being ready to change. If you and your client will try another round of CBT, how can you ensure that your exposure or behavioural experiments are optimised especially when there are no feared consequences or there is a need to tolerate uncertainty?

Severe OCD may also be complicated by various co-morbidities especially depression; trauma or aversive memories; alcohol or substance misuse; low body weight; sleep chaos; ASD; prescribed medication; severe avoidance; over-valued ideation and family accommodation. Approaches to these problems will be highlighted with clinical examples.
